INTRODUCTION

1.1 Introduction

Sayano-shushenskaya is the 6th largest power plant in the world that located in Russia.In the year of 2009, the power plant turn on in disaster because of a troublesome turbine No.2 that broke apart violently. The turbine hall and engine room was flooded , the ceiling of the turbine hall collapsed, 9 out out of 10 turbine damaged and 75 people was death. The disaster also effect on power failure and power loss. This report prepared to determine the root cause of this problem occur, and analysis the way of prevent. By this we can know the proper way to conduct a proper management and things that we should do and do not.

CHAPTER 2

ABOUT SAYANO-SHUSHENSKAYA

2.1 Industrial process

Sayano-shushenskaya hydroelectric power plant that have build in the year of 1978 have the length of complete of 1066m and 80 story high that is 242mabove the river floor. The width of this dam is about 105.7m at its base and 25m on the ridge, the dam is in arch-gravity type and can with stand for earth quake up to 8th ricthed. This plant running on 10 turbines included the troublesome turbine 2. This plant can push to a maximum limit for the production up to 6400MW.

2.2 Management operation

The plant is located on the river of Yensai near Sayangogrsk in Khakassia wich run by Rush hydro with 10 turbines in full capacity all the time and have win a place in a World Record for a largest out put. The disaster happens in the year 2009, when the turbine number 2's vibration lead down to the mounting and the cover of the turbine. The 1100tonnes turbine 2 roes out his housing when the distant signal opens the flow gates and let the water to flow more to spin the turbine more faster. They called it as a cyberatact that mean a friendly fire but actually its from a bad computer command that become a root cause by an improper management

2.3 About The turbine 2

This turbine 2 issued up with its 1st problem on the second year of the plant start run itself that's on the year1980. Many more problems that have been found in this turbine seals , turbine shaft , bearings and in the end of mac 2009 , a complete reconditioning of turbine 2 took place .the turbine also founded degraded vibration and shakes caused by water flow pulsation , it because the working band at high efficiency was narrow.

FALIURE FACTORS

3.1 Managment and technical

Poor management and technical fouls are the main reason for this disaster takes place. Managements poor decision to run the plant in full capacity although there is some problem issued because there was a failure on the nearest plant , so they wanted to backup the power generate. It this case , they have skip some of the start up process to run back the break down turbine faster and also cut down some cost on maintenance , safety, investment and educational. In this case , they suppose to be little selfish by protect their plant by running on the safe mode and put a very good inspection before run back an idle machinery . The turn over by chasing the production may raise the parameter of profit , but if its turn on to a tragedy , its gona be multiple lost like how Rush hydro go through now.' Running the plant in a maximum level also become an fact for this disaster. When all the turbine was running with full capacity, there will be more pressure pump in. Rush hydro already know that there was some displacement on the wall the dam, so they should not approve to run the plant in full capacity. In a proper way , they suppose to have an back up turbine that idil for maintenance , and reconditioning , once some of the turbine get trouble, this back up turbine must run to ensure the out put level maintaine.

Besides that , the management have a poor supervision in current situation , its shows when they did not run on the test frequently for the safety automatic flow gate that is very important to safe our soul(sos) , although its just five months after implementation together with auto speed control. Actually this gate cant closed because the place was in pitch black after the turbine 2 rouse up and hit the wall. So they don't have the power to generate the gate to get closed. When its come to safety gate . it suppose to be in an individual power supply from an generator that still can back up although the entire dam turn black out so it may help to control the water flow in the system to never reach into the turbine house and by making the condition get worst.

When we talk about the auto speed control, the speed of the turbine spin will be controlled automatically depends on the out put signal , if the out put below , it will direct send information to increase the speed of turbine. In this case the signal will be given based on the out put level and
